To better serve our community and reduce long wait times, we are offering two easy ways to get your student-athlete's sports physical completed!

📋 Requirements:

Grades 6th–12th

Completed physical form signed by a parent or guardian

A guardian MUST be present

Choose the option that works best for you:

📞 Option 1: Call for an Appointment

When: Monday – Saturday | 8:00 AM – 7:00 PM

Where: Our Acute Clinic

Phone: (505) 287-6500

Cost: Use your insurance OR a cash fee of $25

🗓️ Option 2: $1 Sports Physical Event

When: July 31st | 1:30 PM – 6:00 PM

Details: Patients will be seen on a first-come, first-served basis.

Cost: Only $1!
