📋 Requirements:
Grades 6th–12th
Completed physical form signed by a parent or guardian
A guardian MUST be present
Choose the option that works best for you:
📞 Option 1: Call for an Appointment
When: Monday – Saturday | 8:00 AM – 7:00 PM
Where: Our Acute Clinic

Cost: Use your insurance OR a cash fee of $25
🗓️ Option 2: $1 Sports Physical Event
When: July 31st | 1:30 PM – 6:00 PM
Details: Patients will be seen on a first-come, first-served basis.
Cost: Only $1!

Grants Cibola County Schools, in partnership with SFE, is offering free summer meals for all children ages 1–18 at multiple sites across the district. Breakfast and lunch are available throughout June and July, with times varying by location.
There is a short form to complete at your first visit, and children must be present only at the first pick‑up to receive meals.
