FREE Flu shots for students. If you would like your child to receive a flu vaccine at school, Please fill out the form using the link provided or a paper consent form that will be sent home with your student. One form must be filled out for each child. The deadline for the form to be completed is October 25th. The vaccinations will occur the last week of October and the first week of November. Thank you.
